Program of Assertive Community Treatment (PACT)

 

PACT services promote independent living in the community for persons with the most severe and persistent mental illness. A multidisciplinary team provides services 24 hours per day, seven days per week.  All services are geared toward increasing tenure in the community, stabilizing psychiatric symptoms and improving the quality of life for identified high need, high-risk consumers. 

Services

Community mental health services provided by the PACT Team are:

  • 24-hour, 365-day-a-year services
  • Intensive case management and
  • Mobile outreach and home visits
  • Psychiatric and nursing services
  • Residential skill training and supports including budgeting, grocery shopping, transportation, activities of daily living, housekeeping, social skills development
  • Substance abuse assessment and treatment
  • Employment assistance
  • Crisis intervention
  • Short-term, focused therapy and supportive counseling

Eligibility

Eligibility for PACT services includes:

  • Arlington County resident, 18 or older;
  • Severe and persistent mental illness: AXIS I diagnosis of schizophrenia, schizo-affective disorder, bipolar disorder, or other psychotic disorder;
  • History of multiple hospitalizations;
  • Difficulty managing many spheres of independent living such as housing, food preparation, home maintenance, finances, employment, and/or physical illness.
