Summer Camp Registration
2013 Summer Camps Registration Form- Formulario de Inscripción para campamentos de verano del Condado de Arlington 2013
- Before your child may attend camp, you must complete all required forms. Forms may be completed and submitted online. Go to: https://egov.arlingtonva.us/campforms to complete and submit forms online. If you do not wish to use our online forms submission process, forms can be downloaded below.

- School Entrance Health Form (Tot Camp only)
- List of Schools / Communicable Disease Reporting (Tot Camp only)*
*Proof of child's identity and age is required for Tot Camp only. Please submit in person to our registration office one of the following documents: certified copy of a birth certificate, birth registration card, or passport (current or expired). Medication Forms
- Authorization for Medication - This form is for a child attending a camp for two weeks or less. This may be completed by the parent.
- Written Medication Consent Form - This form is for preschool for the entire school year or any camp if the child is attending more than two weeks in the summer. This must be completed by a doctor.
