Listing of any non-County resources does not imply an endorsement by Arlington County and is simply for the convenience of our community members in need of non-emergent medical care not appropriate for 911 emergency medical resources.

Nursing Case Management (NCM)

  • The NCM program helps individuals improve or maintain their health so they can remain at home safely for as long as possible.
  • Case management and coordinating health care
  • Assessing and monitoring your health status
  • Pre-pouring your medications
  • Providing health education
  • Referring you for other services you might need

Qualifications 

  • Arlington residents over 60 with multiple chronic illnesses
  • Adults 18-59 with permanent disability who: 
  • require assistance
  • lack sufficient support
  • have a physician to order services
  • have no access to another source
  • no longer eligible for skilled care

  • Adults 60 and up who have physical or cognitive disabilities that makes it difficult for them to complete home/self care
  • Home bound and cannot leave their home
